The Continuum of Care (CoC) Program is designed to promote community-wide commitment to the goal of ending homelessness; provide funding for efforts by nonprofit providers, and State and local governments to quickly rehouse homeless individuals and families while minimizing the trauma and dislocation caused to homeless individuals, families, and communities by homelessness; promote access to and effect utilization of mainstream programs by homeless individuals and families; and optimize self-sufficiency among individuals and families experiencing homelessness.

Homeless Coordinated Assessment Team (H-CAT)
The goal of homeless coordinated assessment is to prioritize persons/families based on needs. Many times people need assistance for a brief period due to job loss, health issues, or other life-changing events. Often these events can lead to a homeless event. The NC-509 uses a system of evaluation and prioritization with an Emergency Response Survey Tool (ERST), a questionnaire designed to quickly assess resources and supports already in place for a person or family. The second step is a much more in-depth survey called the Vulnerability Index – Service Prioritization Decision Assistance Tool (VI-SPDAT).
The Salvation Army and The Shelter of Gaston County, are currently the only agencies providing this service. It is the goal of the Gaston-Lincoln-Cleveland Continuum of Care to use a “no-wrong door approach” to assist persons experiencing homelessness by enlisting community agency support through training, communication, and collaboration.

Veteran Initiatives
NC-509 CoC is served by several agencies providing direct services to Homeless Veterans and their families. Supportive Services for Veteran Families (SSVF) programs are administered by ABCCM, Community Link-NC, and Family Endeavors.
